At the start of the outbreak in 2013, wild poliovirus was considered eradicated in all but three countries: Afghanistan, Nigeria, and Pakistan. The virus traveled from Pakistan to Syria and Iraq in October 2013, where armed conflicts hindered vaccination efforts and facilitated an outbreak. The WHO declared the event a PHEIC at the end of April 2014. Since 2015, wild poliovirus cases have been confined to Afghanistan, Pakistan, Mozambique, Nigeria, and Malawi. Circulating vaccine-derived poliovirus has spread in countries throughout the Middle East, sub-Saharan and central Africa, and southeast Asia. As of 2024, the WHO Emergency Committee on Poliovirus has continued to issue recommendations for countries at different levels of risk. These include increasing routine immunization coverage, encouraging vaccination for travelers and migrants, bolstering case detection and surveillance efforts, and increasing regional cross-border cooperation.
